Thank you for your interest in the Eastern Star Masonic Home. You may begin the application process by downloading the Admission Application, Financial Statement and Medical History Form. Once these forms have been completed please send them to:

The Medical History Form should be completed by a physician 30 to 60 days prior to entering the Home and MUST include the results from a Mantoux Test or a chest x-ray. Once these application forms have been received, you will be contacted by the Home's Social Worker. Part of the application process is to set up an appointment for a tour and interview with each new applicant before final approval is given.
At the time of the interview, additional informational materials will be provided. Once a decision has been made to reserve a room, additional information will be gathered as well as supplemental forms will need to be completed.
